A clinical analysis of periampullary cancer.


Abstract
Clinical analysis of 42 cases of periampullary cancer which were treated at Dept. of Surgery, Medical College, Chung-Ang University from June 1968 through April 1982 was done and the result were as follows
1. 61.9% of all periampullary cancer were of pancreas head, 30.9% were of distal CBD, 4.8 % were of ampulla of Vater and 2.4 % were of duodenum.
2. The sex ratio of male to female was 1 to 1.1 and the most common age group was sixth decade.
E 3. The frequent clinical symptoms were right upper quadrant abdominal pain (69.0%). jaundice (47.6%) and weight loss .(31.0%).
4. The common physical findings were jaundice (69.0%), right upper quadrant tenderness (57.1 %i and hepatomegaly (50.0%).
5. The duration of illness was within 6 months in 80.8 % of all cases.
6. Elevated bilirubin level was noted in 65.956, increased . alkaline phosphatase in 82.996, leukocytosis in 59.596, positive stool occult blood in 52.4%, increased fasting blood sugar in 50.8% and reversed A/G ratio in 45.096.
7. The definitive diagnosis was made pre-operatively in 11 cases (26.2%) using the PTC and ultrasound.
8. Curative operations were performed on 5 cases, palliative operations on 25 cases and biopsy only on 6 cases.
9. The post-operative complication rate was 47.2%. The common complications were hepatic failure, wound infection, bile leakage and pulmonary complication in decreasing order.
10. The post-operative mortality was 13.9%(5/36), one death among 5 radical surgery, one death among 25 palliative surgery and 3 deaths among 6 biopsy only cases.
The main cause of death was hepatic failure.
